Star Polisher Recognition Program

This is an opportunity for staff, parents, students and others to recognize staff members and school volunteers who go above and beyond the normal job responsibilities and expectations.  Documents are provided in Adobe Acrobat Format.

Please use these forms to nominate individuals.  Completed forms should be submitted to the school principal.
The committee will meet in early November to review nominations and select the first group of recipients.
