Requirements
To Attend USD 261
Pre-school: if eligible, 3 or 4 years on/before August 31
Kindergarten: five years old on/before August 31
First Grade: six year old on/before August 31
All new students entering Haysville USD 261, including pre-school and kindergarten, must have a certified copy of their birth certificate on file and a Kansas Certificate of Immunization. Students will be identified by their social security number. Please bring the student’s social security card when enrolling. New to the district students must also show proof of residency. Acceptable proof of residence includes a current utility bill showing the student’s address or a current rental or purchase agreement. Questions about residency should be referred to Debbie Coleman at 554-2200.
Student Immunization Information
Kansas requires all children entering school for the first time in Kansas to provide their immunization records prior to admission to and attendance at a public school. A health assessment is now required for school entry for children through 8 years of age who have not previously enrolled in a Kansas school.
Immunization requirements for 7-18 year old students:
(1) Four doses of DTPaP/DT (diphtheria, tetanus, pertussis) with at least four weeks between each of the first three doses and at least six months between the third and fourth doses. If the last dose was given before the fourth birthday, one additional dose is required;
(2) Three doses of Polio vaccine with at least 4 weeks between each dose, with one dose given on or after the fourth birthday. Four doses of Polio vaccine will be accepted if there is four weeks between each dose, regardless of age;
(3) Two doses of the MMR (measles, mumps, rubella) on or after the first birthday. There should be at least a one month interval between injections.
Hepatitis B: Kansas recommends the Hepatitis B vaccination series for all children through age 17 years old. Three Doses are needed to complete this series.
Children 15 months to 4 years of age
All children 15 months to 4 years of age must provide proof of having received at least the following immunizations prior to admission to and attendance at a public school:
(1) Four doses of DTP/DTaP/DT (diphtheria, tetanus, pertussis) with at least four weeks between each of the first three doses and at least six months between the third and fourth doses;
(2) Three doses of Polio vaccine with at least four weeks between each dose;
(3) One dose of the MMR (measles, mumps, rubella) on of after the first birthday.
Hepatitis B: The hepatitis B vaccination series is required for kindergarten and first grade.
Varcella: The Varcella vaccine is required for kindergarten and first grade entry for 2005-2006 school year. It must be given on or after the first birthday. It is not needed if Varcella disease if verified.
Students Currently Enrolled:
Students who need their 10 year TD (tetanus, diphtheria) booster for school entry for the 2007-2008 school year at the Middle School and Campus High School have been notified by mail. Students will need this booster prior to admission to school.

PHYSICAL REQUIREMENTS
IN KANSAS
Any student planning on participating in competitive athletics is required to have a physical examination recorded on the Kansas State High School Activities Association physical form prior to trying out for any athletic team, cheerleading, and drill team. Physicals must be performed every school year. Forms are available at the Middle School and High School.
