Immunization Requirements for School Admission

Immunizations

Kansas requires all children entering school for the first time in Kansas to provide their immunization records prior to admission to and attendance at a public school.

 

For the 2012-2013 School Year the Following Is Required:

 

A health assessment is required for school entry for children through 9 years of age who have not previously enrolled in a Kansas school.


Immunization Requirements for 5-18 year old students:

1) Five (5) doses DTaP/Tdap (Diphtheria, Tetanus, Pertussis) *A single dose of Tdap is required for Grades 7 through 10 if no previous history of Tdap vaccination, regardless of interval since the last Td. Tdap is required for Grades 11 and 12 if more than 10 years since previous DTaP.
 
2) Four (4) doses Polio (IPV or OPV Schedule)
3) Two (2) doses MMR (Measles, Mumps, Rubella)
4) Two (2) doses

Varicella  (required for Kindergarten through Grade 3 and Grades 7 through 8)  One (1) dose for Grades 4 through 6 and Grades 9 through 12.
None needed if Varicella disease verified as follows:

A) Signature of physician indicating history of disease and date of illness; or
B) Laboratory evidence of Varicella immunity; or

C) Without a physician signature, vaccine is required even if you believe your child has had chickenpox disease.

 
5) Three (3) doses Hepatitis B (required for Pre Kindergarten through Grade 12)

Immunization Requirements for Pre-School:

1) Four (4) doses DTP/DTaP (Diphtheria, Tetanus, Pertussis)
2) Three (3) doses Polio
3) One (1) dose MMR (Measles, Mumps, Rubella)
4) One (1) dose

Varicella.

None needed if Varicella disease verified as follows:
A) Signature of physician indicating history of disease and date of illness; or
B) Laboratory evidence of Varicella immunity.

5) Three (3) doses Hepatitis B
6) Three (3) doses Haemophilus influenzae type b (Hib)
7) Four (4) doses Pneumococcal conjugate (PCV7)
8) Two (2) doses Hepatitis A

Students Currently Enrolled:

Students who need their Tdap (tetanus, diphtheria, pertussis) vaccine or other immunizations for school entry for the 2012-2013 school year have been notified by mail. Students will need the required immunizations prior to admission to and attendance at school.

Updating Student Health Records

Written verification of the immunization must be recorded and a copy provided to the school. Verbal verification is not acceptable. If you have a record that verifies your student has had the immunizations, you may fax that verification to us at 554-2230 or you may send the record with your student to school to give to the Nurse or Health Aide.

Please send verification of all immunizations, dental exam, and vision exam that your child receives over the summer to the school, so their school health record can be updated.
